The deciding cues are a fetal heart rate (FHR) that falls to 70/min as the membranes rupture and stays there, and a pulsating loop of cord felt ahead of the fetal head. The cord has prolapsed and is being compressed. The first nursing action is to lift the presenting part off the cord with a gloved hand and keep it there. The nurse stays with the client and calls for help from the bedside. If the FHR does not recover, the client is helped onto hands and knees with the head down and the buttocks up, and the team prepares for an emergency cesarean birth. The cord is not pushed back in. Amnioinfusion is an option for variable decelerations after the membranes rupture; once the examination finds a prolapsed cord, the plan is an urgent cesarean birth. Oxytocin is reduced or stopped for variable or prolonged decelerations.
Takeaway: with a prolapsed cord, lift the presenting part off the cord, call for help without leaving the client, and prepare for an emergency cesarean birth.
Why each option is right or wrong
- Blank action: Key: lift the presenting part off the cord. Taking the pressure off the cord is the first nursing action.
- Blank plan: Key: an emergency cesarean birth. The team prepares for it while the presenting part is held off the cord.
- an amnioinfusion: Incorrect. Amnioinfusion is an option for variable decelerations after the membranes rupture; once a prolapsed cord is found, the plan is an urgent cesarean birth.
- an emergency cesarean birth: Correct. After the presenting part is lifted and help is called, the client is prepared for an emergency cesarean birth.
- labor augmentation with oxytocin: Incorrect. When variable or prolonged decelerations occur, oxytocin is reduced or stopped, so starting it does not fit this client.
- lift the presenting part off the cord: Correct. With a gloved hand the nurse lifts the presenting part off the cord and keeps it there.
- push the cord back above the fetal head: Incorrect. A prolapsed cord is not pushed back in. An exposed cord is kept warm and moist, with no pressure on it.
- go to the nurses' station to call for help: Incorrect. The nurse stays with the client, keeps the presenting part off the cord and calls for help from the bedside.

- Fill the blank you're surest about first; it narrows the rest.
- Extra choices are deliberate distractors. Don't try to use every one.
